MEDICAL SOCIAL WORKER

Medical Social Worker services:

  • Assisting patient understand financial and emotional resources available to them
  • Assisting with patients discharging form hospital, clinic, or nursing home setting, to return home.
  • Assessment of the social, cultural, religious and emotional factors that contribute to the patients’ response to illness
  • Coordinate the care of patients with complex needs that require multiple services over time
  • Coordinate care for patients and their families with available Healthcare resources
CERTIFIED HOME HEALTH AIDES

CERTIFIED HOME HEALTH AIDES

Cartified Home Health Aides:

  • Assist with Ambulation
  • Assist with bathing and dressing
  • Assist with Mediation Reminders
  • Assist with personal grooming & hygiene needs
  • Assist the RN with checking pulse and breathing rates
  • Provide support for Activities of Daily Living (ADL)
  • Assist with Transfers, including Hoyer lift
